ON THE CURRENT STATE OF MARINE MEDICINE AND ON THE CONCEPTUAL APPROACHES TO ITS DEVELOPMENT IN THE RUSSIAN FEDERATION

Abstract

The paper addresses the problematic issues of military medicine as a health care segment intended for the maintenance and improvement of health conditions of navy personal and of the general populations of seaside areas. The concept of marine medical potential is introduced, and its place in and significance for the general marine potential of Russia are determined. Emphasis is put on the developmental guidelines stipulated in the Project of the Concept of Marine Medicine in the Russian Federation for up to the Year 2030, including the establishment of National and Regional Centers of Marine Medicine. The tasks to be fulfilled by these centers and the requirements to them are characterized. Information is presented about the activities of Marine Medicine Department of the Council of Scientific Experts of the Marine Panel under the Government of the Russian Federation. In 2017, special efforts should be dedicated to establishing of Marine Medicine Departments of Marine Councils under administrations of 21 seaside subjects of the Russian Federation.

CARDIOPROTECTIVE POSSIBILITIES PRO-HYPOXIC PRECONDITIONING

Abstract

Ischemia/hypoxia is an imbalance between the delivery of oxygenated blood to the myocardium and its oxygen demand. The metabolism of cardiomyocytes plays a definite role in the pathogenesis of heart failure, determines the development of such direction in the treatment of this syndrome as cardiocytoprotection. Review is dedicated to the effects of short-term and long-term hypoxic influence on locomotor cells in vivo and vitro. Historical background of the study of hypoxia and hypoxic conditions and pathophysiological mechanisms of hypoxia effects on the myocardium are briefly discussed. Examples of activation of the genetic apparatus in response to ischemia, some physiological changes at the tissue and cellular levels are given. The issues of possible defense mechanisms and activation of adaptive processes in response to subthreshold and threshold hypoxic influence are considered. The authors conclude about the prospects of the search techniques, which have a mild pro-hypoxic effect in the treatment of coronary heart disease.

THE MEDICAL ASPECT OF «STRATEGY-2020»

Abstract

The medical aspects of the realization of the social development strategy of Armed Forces of the Russian Federation are considered as they related to indicators authorized by the decision of the College of the Ministry of Defense of the Russian Federation in 2008. Changes in the indicators that reflect processes taking place among military personnel at service in the European North of Russia are addressed. The indicators include long-term changes in health conditions, traumatism, mortality patterns, medico-psychological rehabilitation, and satisfaction with health care services among military personnel and their family members. Prognoses concerning the indicators are suggested.

A METHOD FOR PRE-TRAINING OF NAVAL CREWS TO WORKING IN SEALED SPACES FILLED WITH HYPOXIC FIREPROOF MEDIA

Abstract

Hypoxic conditioning of special task-force personnel was carried out using an original approach based on biological feedback under conditions of using helium instead of nitrogen for oxygen dilution. Conditioning was carried out prior to placing test subjects into an artificial hypoxic fireproof medium in a pressurized sealed chamber. The hypoxic breathing medium consisted of 93% v/v He and 7% v/v O2 in the conditioning phase and of 70% v/v He and 30% v/v O2 in the restoration phase. Test parameters were the times required to normalize heartbeat, blood hemoglobin oxygenation (SpO2) measured by pulse oximetry, physical performance, anaerobic threshold, visual functions assessed be spatial contrast sensitivity, and sensorimotor responses. Positive changes in the restoration of all test parameters after acute hypoxia were found. Functional performance assessed as suggested by T.D. Loskutova was improved by 15,1%, and simple visuomotor responses were accelerated by 6,95%. The approach is recommended for further developing of targeted particular techniques of discontinuous hypoxic conditioning.

Abstract

The blast trauma on Navy occurs both in military, and in a peace time. The defeats by various factors of blasts are possible at personnel being on ships, afloat, under water, on shoal. The mine blast wounds and injuries, the particular case of blast trauma, are highly actual. When rendering the medical-sanitary aid to defeated at blasts is required the differentiated and pathogenetically reasonable medical and diagnostic approach. In article are described the algorithms of a therapeutic aid to defeated at blasts on Navy. The materials of article were included into new textbooks on naval surgery and naval therapy published in Russian Military Medical Academy in 2015.

TESTING THE TOXICITY OF COMBUSTION PRODUCTS OF MULTICOMPONENT POLYMERIC MATERIALS USED IN SHIPBUILDING

Abstract

Experiments were carried out to assess the toxicity of combustion products of materials used in shipbuilding for decorative and finishing purposes and for furniture manufacturing. The toxicity of combustion products of poorly inflammable laminar plastics compositions, polyurethane, and combinations of laminar plastics and polyamide films has been shown to increase at temperatures ranging from of 600° C to 800° C. The results show that testing the intact forms of polymeric materials as well their forms included in compositions is warranted.

Abstract

In the history of development of the Russian Far East Vladivostok naval hospital it occupies a special place, since it begins with the development of the Far Eastern medicine during the period of political transition and geographical discoveries in the XVIII-XIX centuries. This is due primarily to the development of Russian naval forces in the Pacific Ocean, a change of location of the main port of the Pacific Fleet, and with it - and the Naval Hospital from Okhotsk - Petropavlovsk-Kamchatsky - Nikolaevsk-on-Amur - Vladivostok. The 1477 Naval Clinical Hospital in 2017 will celebrate its official anniversary - 145 years, however, the author cites the documents covering the earlier history of this hospital.

Abstract

The history of establishment of Clinical Navy Hospital No 1469 (CNH 1469) of the Ministry of Defense of the Russian Federation starting from 11 June 1936 is described. The harsh northern conditions promoted the development of the traditions characteristic of the personnel of CNH 1469, such as selflessness, enthusiasm, consideration, and professionalism. The expanding responsibilities of the Northern Fleet call for further perfection of the operation of the Hospital. At present, CNH 1469 is a potent therapeutic and preventive facility, which successfully fulfils tasks related to the maintenance and improvement of health conditions of Northern Fleet personnel.
