Abstract

The article provides an overview of the results of research devoted to climatic and geographical factors of the Arctic. From the standpoint of environmental physiology conducted analysis of the peculiarities of human exposure as a specific (changes of photoperiodism, the characteristics of electromagnetic radiation and fluctuations of atmospheric pressure) and non-specific (cold, low and high absolute humidity, specific aerodynamic mode) factors in Arctic. Article substantiates a conclusion that the Arctic territory rightfully can be attributed to the extreme zone on a number of parameters that more demanding to the functional systems of the body, reducing the reserve capacity of the organism, complicating the work and life of residents.

Abstract

The analytical review presents the data of foreign and domestic literature devoted to modern concepts of risk factors for Clostridium difficile (Cl. difficile) infection-associated. Particular attention is paid to the key links of the pathogenesis of Cl. difficile-associated diarrhea, colitis (including pseudomembranous and haemocolitis) — violation of the bowel microecosystem resulting from the use of antibiotics or anticancer and other drugs with antimicrobial activity, playing the role of the trigger level, breaking mikroekosistemu and amino acid composition of the intestinal environment and create selective advantages for the colonization of the intestine of toxigenic strains of Cl. difficile. Results of work demonstrates the importance of other risk factors for Cl. difficile-infections: age, radiation therapy, surgery on the organs gastrointestinal tract, illness or injury leading to disruption of blood flow and ischemia of internal organs, prolonged patient hospital stay and immunosuppression.

NEURAL CONDUCTION AT TOURNIQUET TEST

Abstract

We review data available on short-term local ischemia on nerve conduction in humans; diagnostic value of this test and its significance in normal and pathological condition are evaluated. Different methods of local ischemia are reviewed; main trends of neuronal conduction changes in normal and pathological conditions are summarized. Are known experimental studies in which to assess of the reserve possibilities of the peripheral nervous system is used short-term influence of ischemia, so called tourniquet test, on the parameters of the conduct of pulse of the nerve, the excitability of axons. Most clinical and experimental studies of the effect of short-term neural ischemia devoted to the study of excitability of nerves. To investigate of the excitability of nerves used the method of «threshold tracking». However, the conventional method of determining of the neural conduction in clinical practice is the simplest, least expensive and also reflects of the resistance to local short-term compression of nerves in diabetes mellitus, acute inflammatory demyelinating polyneuropathy. Tourniquet test in the form of local short limb ischemia of the peripheral nerves in normal conditions leads to a natural reduction in neural conductivity and can be used as a method of standardization and regulation of neural conduction parameters of the peripheral nerves. The degree of reduction of neural conduction depends on the duration of the compression, place (shoulder, forearm), cuff width, age. Tourniquet test has a diagnostic informative in assessing of the reactivity of neural conduction, because the nerve fibers in various pathological conditions are resistant to local ischemia.

Abstract

The use of laboratory information systems (LIS) are now an important part of the activities of clinical diagnostic laboratory of modern hospitals. The results of the work on the creation in 2002 and implementation in practice of clinical diagnostic laboratory multi-departmental hospital LIS. Upgrading the system and add-ins carried out in 2005, 2008, 2010 and 2012. The system is designed for lab work thresholds from 200 to 1000 samples per day, which this LIS operates clinical laboratories in the normal mode, which confirmed the successful implementation of the LIS-1477 in the laboratories of hospitals years. Fokino, Spassk-Dal’niy, Dalnegorsk, outpatient department VMKG. Using the LIS-1477 changes the principle of the clinical diagnostic laboratories to optimize all the effort, organize, standardize and simplify workflow. Promising areas for improvement of the system described in this section is to create a «quality control», as well as interfacing with automated laboratory analyzers. This will automatically transmit orders to the analyzers and obtain their research results without manual data entry.

THE EFFECTS OF AVIATION NOISE ON THE AUDITORY FUNCTIONS IN THE MEMBERS OF FLIGHT AND TECHNICAL PERSONNEL OF CARRIER-BASED AIRCRAFT

Abstract

The objective of the present work was to study the effects of aviation noise on the auditory functions of the personnel of carrier-based aircraft during long-term trips of ships equipped with aviation. The adverse effects of aviation noise on flight and technical support personnel were found to include significantly compromised functions of the auditory analyzer, mainly among the support personnel. The findings warrant the development of more effective individual means of protection against aviation noise and of in-trip rehabilitation programs and procedures to be exercised with account of specific disturbances found in auditory functions.

Abstract

The present article presents data relevant to addressing the issues of medical and biological waste handling in civilian and naval health care and on board of navy ships in the Russian Federation. It is shown that many of the juridical and technological issues of on-board treatment of medical and biological waste generated during regular exploitation of vessels and upon possible emergency situations are still unresolved in the Navy of the Russian Federation. The analysis of available data suggests that the technologies of waste neutralization, elimination, storage, and deposition currently used in the health care system of the Russian Federation are suitable in the Navy.

Abstract

The factors of ship habitability are discussed based on data of sanitary and epidemiological expert assessments related to of technical projects and performed at the stage of preparation to state testing. The subjective estimates of ship habitability presented by the members of ship crews are compared with disease incidences among them. Patent search data related to the automated management of human health risks are compiled. A useful model developed by the present author is described. The model relates to automated assessment of health risks of the members of the crews of anticipated ships and boats.

Abstract

The essay addresses a little-known episode of the World War I: the defense of Petrograd against assault coming from the Baltic Sea. Relevant information about Peter the Great’s Naval Fortress and its strategic significance is presented with emphasis on its medical (sanitary) services. Personal data about medical officers of the Naval Administration, Baltic Fleet, and the Fortress and its port are compiled. The essay is intended to open the series of publications dedicated to the medical service of the Fortress, including medical facilities of Revel (Tallinn), Helsingfors (Helsinki), Vyborg, Libava, Kronstadt, and Petrograd, the medical troops of land forces of the Fortress and of Baltic Fleet ships, and of the hospital ships of the Fortress. The principal objective of the investigation is to reconstruct the names of medical doctors and nurses in service of the medical facilities of the Fortress.

PROFESSOR V. G. CHVYREV CONTRIBUTION IN RESEARCH ON SUBMARINE HABITABILITY

Abstract

The artcle is devoted to the naval hygienist professor Victor Georgievich Chvyrev. His scientific and pedagogical activity amounts more than 50 years. His manyfold investigations on submarine habitability factors as well as his elaboration of well-grounded hygienic standards of habitability conditions are his main contribution to the development of naval hygiene and medicine. A lot of cientific and personal credit must go to Chvyrev efforts in changing the High Naval Command attitude to submarine habitability problem as well as to preventing measures concerning submarine accidents. Owing to his activity the habitability conditions are regarded as a significant feature of ship fighting capasity. A number of naval and radiation hygiene specialists are trained under V. G. Chvyrev scientific supervision, and all made a significant contribution to the development of hygiene — the basic preventive and medical science.

Abstract

The present article is dedicated to the graduate of Emperor’s Military Medical Academy, marine doctor N. A. Novikov and addresses his medical and research activities when he was on service in Kronstadt at the Her Emperors Majesty Great Princess Alexandra Aleksandrovna’s Fifth Crew. Due to his efforts, dentistry rooms were organized at ship squadrons and on Class I vessels, including the cruiser «Aurora». In 1907, N. A. Novikov issued his monograph «Brief Notes on Dental Diseases and Cure Thereof». N. A. Novikov delivered lectures to medical assistants at Nikolayevskiy Naval Hospital in Kronstandt.

THE EXPERIENCE OF HEALTH CARE PROVISION TO THE MIXED CREW OF INS VIKRAMADITYA AIRCRAFT-CARRIER DURING SEA TRIAL

Abstract

The organization of medical services and health care during the sea trial of INS Vikramaditya aircraft-carrier is analyzed. The ship is the product of modernization of the aircraft-carrying cruiser «Admiral Gorshkov». The history of her building and exploitation is briefly discussed. This is the first  time in Russian shipbuilding that such a large-scale project has been implemented for a foreign state. Therefore, highly specific objectives of comprehensive health care faced the medical services of the Northern Fleet. The main difficulties and particular features of sustaining, developing and organizing the medical and sanitary services included the mixed personnel of the crew involved in servicing the ship during her sea trial and the non-departmental status of the ship and of the civilian part of the commissioning team. Specialists from all over Russia were recruited to the modernization of «Admiral Gorshkov» aircraft-carrying cruiser. Evacuation and in-hospital treatment of patients having somatic and infectious diseases were coordinated with civilian medical services. The organization and provision of health care related to large-capacity vessels in the course of their construction warrant specific approaches and the development of novel algorithms of regular medical services, which must be reinforced.

THE SANITARYHYGIENIC AND ANTIEPIDEMIC SECURITY OF «CAUCASUS 2016» WAR GAMES

In September 2016, the main phase of «Caucasus 2016» war games was exercised in Crimea. For the sake of observing the sanitary legislation of the Russian Federation, surveillance measures were taken by the personnel of SESC 636. Due to large-scale preventive interventions, there were no outbreaks of intestinal infections in the course of the war games.
