Abstract

Motion  disease (naupathia, seasickness) is quite prevalent among otherwise healthy people  and  is manifested as travel sickness associated with  nausea. It  is a complex physiological response to certain motor stimuli caused by  ship  rocking, aircraft  swinging or  zero  gravity.  The  motor stimuli may  be  both  real (ship  rocking) and  virtual (watching a 3D movie).  During the  last  decade, numerous studies addressed different stimuli that can  cause naupathia, and  numerous hypotheses have been  put  forward to explain it (sensory conflict,  postural instability etc.) from  the  point of view  of neural plasticity. However, no consensus concerning the  origin  of the  specific  clinical  pattern of naupathia (nausea, vomiting, drowsiness, dysphoria etc.) has emerged by now. Several drugs and  behavioral recommendations have been  suggested for  prevention and  treatment of naupathia. Further studies of this  condition using such  techniques as videonystagmography and  computer-assisted posturegraphy, as well  as genetic analysis, are  warranted.

TO THE QUESTION ABOUT SICKNESS RATE OF SHIP’S CREW OF MARINE FLEET AT FAR-EASTERN REGION

Abstract

The  paper addressed the  disease incidence of crew  of marine fleet  at Far-Eastern Region  both  by  official accounts, and  by materials of the  profound elaboration, including address of ship’s crew  for medical attendance in trips.  The  long-term investigations on influence of the  professional work,  age, length of service and sex  of sailors,  ship’s environment and  naturals factors, as well duration trips and  ships  of various fixing  on the  health of seamen. By observation were 11 000–15 000 seamen, 80% which  were men  to 40 age. In structure of the  disease incidence predominated illnesses to which  rise  important play  a part cold factor, physical and  nervous strain. Analysis of temporary incapacitation ascertained prevalence at commanding staff hypertonicity, cardiac diseases and  ulcer  of the  stomach, in which  rise  essential role belongs nervous and  emotional  factors. Comparison disease incidence by chief profanations take into consideration principal differences in groups showed, that its higher at persons, which  ensuring speed and  clear  of danger navigation. In consequence ascertained chief  influence professional and  industrial factors on the  state of seamen’s health.

PRESENT-TIME RESOURCES FOR DIAGNOSTICS OF ABDOMINAL WOUNDS AND TRAUMAS UNDER EMERGENCY CONDITIONS IN THE PACIFIC REGION OF RUSSIA

Abstract

The  diagnostic effectiveness of clinical, laboratory, instrumental, and  surgical methods of diagnostics of internal organs injuries upon  abdominal wounds and  traumas is considered. Most  injured patients (68,1%) were found on admission to exhibit «dim” clinical  manifestations of hemorrhage or peritonitis. Only  31,9% of patients exhibit clear  symptoms of injuries of their abdominal organs. However, the  clinical  pictures did not always corresponded to the  symptoms. Overdiagnoses based on clinical data were attributed to 51,2% of cases. The sensitivity, specificity, and  accuracy of ultrasonic diagnostics of intraperitoneal hemorrhages were estimated to be 51,2%, 88,1%, and  80% respectively. The  proposed approach to diagnostics of injuries implies direct laparoscopy when general anesthesia and  the  instrumental methods of investigation are  impracticable. The  benefits of this  approach include its high  diagnostic effectiveness, relative simplicity, minimal demands for  instruments and  equipment, and  the  possibility to carry out  outdoor examinations, if necessary. As an alternative to laparoscopy, laparocentesis may  be employed, if time  is pressing and  facilities are  scarce.

FEATURES OF CONSERVATIVE TREATMENT OF CHRONIC OBLITERATING ATHEROSCLEROSIS OF LOWER LIMB ARTERIES IN THE PRIMORSKY TERRITORY

Abstract

We analyzed the  age  criteria, time  of admission, comorbidities and  structure of chronic obliterating atherosclerosis therapy of lower  limb  arteries. It was  found that the  degree of severity of chronic ischemia of the  lower  limb arteries atherosclerotic lesions  does not correlate with  the  age factor, however, is closely  associated with  the  time  of admission of patients to the  hospital. Early treatment  for  medical patients with III and  IV degree of chronic lower  limb ischemia is associated with  the  presence of this  category of persons expressed pain  in the  legs  and  feet  and  trophic complications, pathogenetic link  which  is circulatory, and then cellular hypoxia, leading to cell death and  tissue up to the  development of focal necrosis. In the  structure of concomitant chronic lower  limb ischemia pathologies dominate diseases of the  cardiovascular system (coronary heart disease and  hypertension) in 100% of patients, nervous system diseases (encephalopathy) in 76,9% of patients, indicating that systematic, generalized atherosclerosis obliterans, which  is characterized by  involvement in  the  pathological process is not  only  lower  limb  arteries, and  distant arterial pools  of organs and  systems. It  has  been  shown that the  combined application of traditional preparations of basic therapy, intravenous infusions of ozonated saline  reopoliglyukina trentalom with  sulodexide and  shown to be highly effective in chronic critical limb ischemia therapy. Patients with  chronic obliterating atherosclerosis of the  arteries of the  lower  extremities is necessary to conduct a life-long therapy with  regular cardiovascular  hospitalizations not  less than 2 times a year for  routine preventive treatment.

Abstract

The  objective of the  present paper is to evaluate the  psychophysiological adaptation of freshmen to studying at Pacific  State Medical  University. The  habituation of freshmen to education is the  first and  most important task  of higher school. The  complexity and  specific  features of medical training present special challenges to  psychophysiological adaptation, which  warrant developing a dedicated training program. Its  outcomes were assessed using parameters that reflect the  general state, activity and  mood  as well as stress resistance and  socialization of freshmen and  subjected to statistical treatment. The  training program was  developed to teach techniques and  methods of self-regulation to freshmen thus making them able  to control their emotional and  physical conditions in professionally and  vitally important situations.

Abstract

The  present review addresses currently available information about securing the  biological  safety of healthcare personnel exposed to pathogenic microorganisms attributed  to pathogenicity groups I–IV.  The relevant measures constitute the  most  important aspect of organizing the  activities of medico-prophylactic institutions. Adequate staging of preventive measures provides for  uninterrupted operation of not  only specific  wards of  a  medical institution but  the  entire institution as  well.  Careful adherence to  simple rules is a prerequisite of appropriate workability of a medical unit.  The  topicality of this  agenda is currently on the  rise  because of the  danger of biological  terrorism.

The  present publication sheds light on  the  operation of Naval  Hospital affiliated with  a remote Vladimiro-Olginskiy Garrison. The  work of medical units of this  remote hospital is described. The  history of the  establishment and  development of the  medical services of this  stand-alone garrison is reviewed with emphasis on provisioning it with  all sorts of allowances. The  topics  addressed include the  life and  work of officers, workers and  clerks,  public  events, research activities, and  healthcare provision to military and common people  in Olginskiy District of Primorskiy Region.

PARTICIPATION OF SANITARY SERVICES PACIFIC FLEET MEDICAL IN SUPPORT OF COMBAT OPERATIONS AT LAKE KHASAN

Abstract

Since  the  time  of the  military border conflict  in a small  Lake  Khasan in the  south of Primorsky region of Russia has  passed 79 years. It is already history, but  she  is always ready to present lessons and  to enrich us the  necessary experience. This  aggression is rightly regarded in history as a serious test  for the  Pacific fleet  and  Far  Eastern Army on the  eve  of the  Great Patriotic War. The  experience of the  Sanitary Service of the  Pacific  Fleet  in the  organization of medical support acquired at Hassan, had  significant implications for  the  next formidable military events. He allowed revising the  basic  principles of medical care,  surgical care  approaching to the  field  of the  bout,  the  organization staged treatment and  specialized medical provision. A small  naval hospital in Vladivostok took the  brunt of the  struggle for the  recovery of the  wounded. Khasan events were remarkable and  heroic  page  in the  history of the  Sanitary Division  of the  Pacific  Fleet.

Abstract

The  article discusses the  history of the  formation of the  clinic  and  the  Department of Otolaryngology Pacific  Medical  University, since  1963 and  the  present day. The  information about the  first head of the department Vitaly  Matveyevich Schevtsove, thanks to the  enthusiasm and  energy that the  clinic started a scientific, methodical, erudite and  educational and  medical work.  The  contribution of the  leaders in the development of the  hospital, its main  stages, scientific achievements, defended dissertations and  published training manuals. Reflected work student circle,  the  possibility of using multimedia technologies, endoscopic and  microsurgical opportunities in the  educational process. It is shown that significant attention in the  clinic  of  ENT  diseases Pacific  Medical  University devoted to  the  provision of  specialized medical care,  a high  level  which  makes it possible to consider this  institution undisputed leader providing ENT care  in the  Primorsky Territory and  the  Far  East.

AN EXPERIENCE OF SECURING THE SANITARY AND EPIDEMIOLOGICAL SAFETY OF THE ARMED FORCES OF THE RUSSIAN FEDERATION BY STATE OFFICERS IN CHARGE OF LAW OF PROCEDURE

Abstract

Measures taken at State Sanitary and  Epidemiological Surveillance Centers of the  Ministry of Health of the  Russian Federation to control the  sanitary and  epidemiological safety of the  armed forces and  general population help  to  reveal administrative offences by  officials  and  juridical entities prone with  hazards for  the  life and  health of military men.  Most  of the  offences are  associated with  services related to the conditions of military barracks and  their vicinities and  to food  processing. The  corresponding penalties according to the  Code of Administrative Offences (COA) include fines  and  warnings. However, more  than 90% of such  cases  are  complained at arbitral or appellate courts. The  present publication presents an experience suggesting how  to  deal  with  such  cases  so as  to  prevent unjustified avoidance and  delays of due  penalties.
